Below is a survey to help advocate for national changes to wheelchair or scooter repair. The consumer must have used Medicare and/or Medicaid for the repair in the lastΒ  three years. See details below:

A group of advocates including the Colorado Cross Disability Coalition and Disability Rights Education and Defense Fund are collecting information about mobility device user experiences getting their equipment repaired. You only need to complete this questionnaire if you have Medicare and/or Medicaid and if your wheelchair or scooter has needed a repair in the last three years. We will forward your responses to CMS. 

This is an effort at the Federal level to point out to the staff at CMS, specifically the Medicare Ombudsperson, that beneficiaries are having a hard time getting repairs of their mobility devices.  The survey is about 10 questions with mostly click the response questions and a couple of questions asking for free text.

Hi! My name is Gina. I had Meningoencephalitis in 2008 and I've been in a wheelchair since then. Meningoencephalitis is a rare and life-threatening condition in which you have meningitis and encephalitis at the same time. Healthcare providers may also call it encephalomeningitis.
